Why intensive nipple cream? How we made ours for Mamas

The story behind Mama's Choice Intensive Nipple Cream: what mothers asked for, what is in the tube, and how to fit it into the early weeks of breastfeeding.

Breastfeeding is meant to feel close and calm. In the first weeks it often feels raw instead.

Most of us on the Mama's Choice team have sat up at 3 a.m. with a baby who wants to feed again and nipples that feel dry, tight and tender. That stage is common, and it usually passes as you and your baby find your rhythm. A good latch does most of the work. A good cream can make the skin feel a great deal more comfortable while you get there.

That is why we made Mama's Choice Intensive Nipple Cream.

What mothers told us they wanted

Before we settled on a formula, we asked mothers a simple question: what would you want from a nipple cream?

The answers were consistent. Something rich enough to stay put on dry skin. Something that did not feel greasy or sticky in a nursing bra. Short, familiar ingredients. And above all, a cream you would not have to wipe off every time your baby wanted to feed, because nobody has a free hand for that at 3 a.m.

Those answers shaped the cream. It took many rounds of testing to get the texture right: thick enough to cushion, soft enough to spread on tender skin without rubbing.

What it feels like to use

The cream is dense and buttery in the tube and melts as it warms on your fingertip. A pea-sized amount covers both nipples. It leaves a soft, protective layer that keeps the skin moisturised between feeds, and it carries no added fragrance, so there is nothing strong for your baby to smell.

There is no need to wipe it off before a feed. Apply it after feeding, or a few minutes before, whenever the skin feels dry.

Mothers who used it told us what they noticed. In a Mama's Choice satisfaction survey of 74 mums in Indonesia in 2022:

  • 96% felt dry nipple skin became softer
  • 93% felt the skin on their nipples looked smoother
  • 98% felt the texture was gentle in use

Results differ from person to person.

What is inside the tube

The ingredient list is short, and it is printed in full on the pack: lanolin, shea butter, coconut oil, vitamin E (as tocopheryl acetate) and date fruit extract.

Lanolin

Lanolin comes from sheep's wool and is the base of the cream. It is a rich emollient that sits on the skin and holds moisture in, which is why it has been the classic nipple balm for generations. If you have ever reacted to wool or to lanolin, patch test first on the inside of your arm.

Shea butter

Shea butter softens dry skin and gives the cream its smooth, buttery feel. It helps the lanolin spread without dragging on tender skin.

Coconut oil

Coconut oil has been used as a skin oil in Malaysian homes for as long as anyone can remember. In the cream it adds slip and leaves the skin feeling supple.

Date fruit extract and vitamin E

Date extract and vitamin E round out the formula as conditioning ingredients that help keep skin feeling soft.

Why the latch still comes first

Most nipple soreness in the early weeks comes from how your baby is attached. A shallow latch pinches the nipple against the hard palate, and no cream fixes that. If feeding hurts all the way through, if the nipple comes out flattened or wedge-shaped, or if pain is getting worse after the first week, ask for help with positioning from your klinik kesihatan nurse, a lactation consultant or a breastfeeding peer counsellor.

See a doctor promptly if you notice a hot, red, painful patch on the breast, a fever or flu-like aches, pus, or bleeding that does not settle. These can be signs of mastitis or an infection, which need medical care.

Used alongside good positioning, the cream is there to keep the skin moisturised and comfortable while things settle. Many Mamas start using it in late pregnancy if their nipples feel dry or itchy, and Malaysian mothers often keep a tube in the diaper bag for dry lips and dry patches too.
